Accident Reports
Within 72 hours of an accident, a supervisor should fill out the Princeton University
Accident/Near Miss Investigation Form, available online from the Library Web
Page at http://libweb.princeton.edu/hr/forms/forms.html.
A copy should then be sent to Dottie Pearson, who will forward it to Greg Cantrell
at the Office of Environmental Health and Safety (EHS) at 262 Alexander Street.
The supervisors should retain a copy for the departmental file.
A copy of this form appears on the next page, followed by instructions for its
completion.
Incident Reports
Incident Reports are filled out by Public Safety, Library Security and Library
Staff as required and are not to be confused with the Accident Reports. Incident
reports are a summation of the problem or incident, which occurred. It is important
to fill out an Incident Report because it allows to us to keep track of various
problems. A patron who may cause a disturbance at one library or work area may
have a history of causing disturbances at other libraries or work areas. Tracking
incidents allows the library to take appropriate action much faster. A copy
of the form is attached and it is also available online at http://libweb.princeton.edu/hr/forms/forms.html.
Branch libraries and Firestone staff should report all incidents to Dottie Pearson
so that appropriate action can be taken. Emergency situations in Firestone should
be reported to Library Security (8-3221) or Dottie Pearson immediately by telephone,
and branch libraries should contact Public Safety (8-1000) if necessary.
